Afib: initiation at the RIPV ➡️ termination at the RSPV as we wrapped up our PVI

#ePeeps check out the initiation of afib/aflutter at the base of the RIPV from sinus rhythm as we were getting ready to complete our pulmonary vein isolation.

After pacing the anterior side of our right veins to avoid making “friends” with the phrenic nerve, we completed our PVI via WACA to have the most glorious #burnandterm at the anterior RSPV.🫀

There’s still so much more to learn about the mechanism of atrial fibrillation, but it’s hard to argue with that juicy signal and having nothing but NSR in the end! 🥳

Cor triatriatum…….Cor triatri-WHat-um? Does double the chambers in the RA = double the fun?

#ePeeps Check out this rare congenital defect we were able to visualize, while preparing for a “typical” flutter CTI ablation line using our ultrasound technology🫀

In homeview on our ultrasound catheter, we see that the extra membrane separating our RA appears to be fully closed and impenetrable. Clocking our catheter around toward the left atrium we spotted and targeted a gap in the membrane for us to access our Cavo Tricuspid Isthmus line. We were even able to confirm the space using the Doppler tool to visualize blood flow from the RA ➡️ RV.

We made our way to the opening in the membrane, and with some finessed catheter maneuvering and candycane-ing our STSF catheter, we completed our line of block to #burnandterm. It’s hard to say “typical” when patient’s care is so specialized, but I’m stoked have tools that allow us to adapt to complexities in real-time.

Post PVI ablation for AFib and checking our work, we wrapped up in the LA and headed to the Right Atrium. The patient’s prior history showed typical atrial flutter so we grabbed an ultrasound contour and prepped for our drive-by CTI line

It was hard not to geek out as we were securing our line and watching all this EP unfold under the RA! Here’s what I was seeing #behindthescreens using #CartoREPLAY

My CS catheter at 3’o’clock on the LAO caudal RA map (correlating to my green signals) was firing on all cylinders as we watched film from our #ablationtermination

Map 1-2 (highlighted in blue on the left) is one of the signals from the PACs we were tagging (green points on my RA map) correlating to AALLL that activity ⬆️ !! We can see our ablation hitting those juicy signals, and tickling that tissue till our #burnandterm
